چکیده

Ulcerative colitis is an inflammatory chronic disease primarily affecting the colonic mucosa. Neutrophils within epithelial structures, such as the crypt wall (cryptitis), or the crypt lumen and wall (crypt abscesses) or in association with crypt damage (crypt destruction) are helpful for the diagnosis. To assess ultrastructural changes of the colonic epithelium in patients with active UC with special referral to the development of crypt abscesses. Methods: a total number of 28 patients, 20 with clinical manifestations of UC and 8 healthy subjects underwent colonoscopy. The biopsies obtained during colonoscopy, after specific preparation were stained using routine H&E histological methods using a light microscope Carl Zeiss Ergaval to establish the histopatological diagnosis. The specimens for ultrastructural assessment were stained with uranyl acetate and lead citrate and studied with a JEM-1010 transmission electron microscope. We observed the rarefaction or even the absence of the microvilli, the reduction of goblet cells, the shatter of the epithelial junctions, cytoplasmic vacuolisation, dilatation of the endoplasmic reticulum, pycnotic nuclei, the destruction of the mitochondria and Golgi complexes which conducts in the end to the drastic reduction of the mucus production. Our investigation has revealed that in acute stage of UC a loose cohesion of epithelial cells is present. We can say that, in the acute stage of ulcerative colitis, a great part of the ultrastructural changes are interrelated with the disorders of the epithelial cells. Cryptitis, crypt abscesses and crypt destructions are important in the development of the inflammatory process in UC.

Histological discrimination of idiopathic inflammatory bowel disease from other types of colitis.

AIMS To assess the value of histology in diagnosing inflammatory bowel disease (IBD) in colorectal biopsy specimens. METHODS Retrospective, double blind evaluation of colorectal biopsy specimens from 41 patients with colitis (28 with ischaemic colitis and 13 with acute self-limited colitis) and 84 patients with IBD (42 with Crohn's disease and 42 with ulcerative colitis). RESULTS The featur...

Challenges in the diagnosis of ulcerative colitis with concomitant bacterial infections and chronic infectious colitis

Ulcerative colitis (UC) is a chronic inflammation of the large bowel characterized by diarrhea and a negative stool culture. However, several enteropathogens have been implicated as causative agents in UC. The differentiation between chronic infectious colitis (IC) and UC with concurrent infection is difficult owing to their similar clinical presentations. The study aimed to explore the present...

Appendiceal Orifice Inflammation Mimicking Submucosal Tumor on Roentgenogram in Ulcerative Colitis

Appendiceal orifice inflammation (AOI) is observed as skip lesion in distal ulcerative colitis (UC). The endoscopic frequency of AOI is reported to be 7.9% to 76% of distal colitis. UC is a relapsing and remitting disease and a morphological change of the large bowel occurs over time. Patient and physician can easily understand the shape of the whole large bowel not through endoscopic photograp...

Mycophenolate mofetil-related pancolitis in a kidney transplant recipient.

Gastrointestinal adverse effects are common with mycophenolate mofetil administration, especially diarrhea. We report a case of mycophenolate mofetil-related colitis in a kidney transplant recipient. Colonoscopy revealed an ulcerative diffuse colitis. The colonoscopic biopsy specimen showed mild crypt distortion, accompanied by cryptitis and focal graft-versus-host disease like changes. The pa...

Exacerbation of inflammatory bowel disease with isotretinoin

A 27-year-old female medical student experienced an episode of intermittent bloody diarrhoea for 6 weeks. She had no prior illnesses, no family history of note and had not undergone any recent travel. Routine blood and stool tests by her GP were normal. A colonoscopy showed patchy moderately active colitis and biopsies revealed patchy mild to moderate chronic active inflammation with cryptitis ...
